Orica Reveals Report Mentioning Cause of Chemical Leak In an independent review, Chemical company Orica has shared the actual cause of the chemical leakage accident occurred last month on August 8. Done by consulting firm Johnson Matthey, the embattled company had been under scanner to ensure that accountability is being set, lack of which is believed to have triggered the leak of hexavalent chromium from Orica's ammonium nitrate plant at nearby Kooragang Island.

The report stated: “The design and operational issues caused the process steam to come into contact with cold surfaces inside the system at lower temperatures than expected”.

The accident caught the company off-guard, and has sparked controversy in the contour of society, thereby questioning the fact that how a three-day delays occurred.

Not only concerned authorities are under the knife, the O'Farrell government was also questioned over the slack measures being implemented, overlooking the need of warning general public about the possible concerns attached with leak of hexavalent chromium within time.

Though no significant concern was notified in the Stockton community, same even confirmed by NSW Health, Premier Barry O'Farrell was being bashed for putting the life of general public on risk.

While the political talks would take its due course of time to die down, the report is expected to be made public next week in community meetings in the nearby suburb of Stockton.
